About Case Management
The SWODA Case Management program serves elderly and disabled people on the ADvantage program in Beckham, Custer, Greer, Harmon, Jackson, Kiowa, Roger Mills, and Washita Counties. The SWODA Case Management service promotes an in-home community based care system and allows frail, at-risk elderly to continue independent living. It provides home visits to assess needs of the individual and works to protect and promote the health and well-being of consumers. In order to qualify, an individual must be Medicaid-eligible for the ADvantage program.
ADvantage
ADvantage* is designed to help the elderly and disabled remain safe in their own homes. ADvantage uses state Medicaid funds to provide services, and eligibility is determined by the Department of Human Services. *ADvantage is a program provided under contract with the OKDHS Aging Services and Oklahoma Health Care Authority (OHCA).
Reduced Cost Eyeglasses Program
Eligible participants can receive reduced rates for eye glasses through participating optometrists every two years. This program is available in Beckham, Custer, Greer, Harmon, Jackson, Kiowa, Roger Mills, and Washita Counties. Eligibility is income based, and if case managed, the participant must be case managed by SWODA.
